（三）主要成分、理化性质、研制方法、生产工艺、质量标准、检测方法、检验报告、稳定性试验报告、环境影响报告和污染防治措施；
（四）国务院农业行政主管部门指定的试验机构出具的该饲料、饲料添加剂的饲喂效果、残留消解动态以及毒理学安全性评价报告。
申请饲料添加剂进口登记的，还应当说明该饲料添加剂的添加目的、使用方法，并提供该饲料添加剂残留可能对人体健康造成影响的分析评价报告。
国务院农业行政主管部门应当依照本条例第九条规定的新饲料、新饲料添加剂的评审程序组织评审，并决定是否核发饲料、饲料添加剂进口登记证。
首次向中国出口中国境内已经使用且出口国已经批准生产和使用的饲料、饲料添加剂的，应当依照本条第一款、第二款的规定申请登记。国务院农业行政主管部门应当自受理申请之日起10个工作日内对申请资料进行审查；审查合格的，将样品交由指定的机构进行复核检测；复核检测合格的，国务院农业行政主管部门应当在10个工作日内核发饲料、饲料添加剂进口登记证。
